Insurance Information

At Oral and Facial Surgery Associates we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this we work with you and your insurance carrier on reimbursement for covered procedures. Please bring your current medical and dental insurance information with you the day of your consultation. This will expedite your appointment and future treatment needs.

Before all surgical procedures are performed our friendly staff will contact your insurance carrier for eligibility and benefit information. You will be given an estimated amount that you will be responsible for the day of your procedure. For your convenience our office staff will also provide you with a copy of the days charges as well as submit those charges to your insurance carrier

Please remember that you are ultimately responsible for all services rendered by this office regardless of your insurance coverage.

Financial Options

For your convenience we accept Cash, Check, Visa, MasterCard, Discover,
American Express and Care Credit. (Care Credit is a credit card for medical or dental procedures that you may apply online or in our office and use at most facilities.)
